Urology

The Department of Urology at Fortis Hospital is dedicated to providing state-of-the-art medical and surgical care in all aspects of adult and pediatric urology. Devoted to complete clinical access in urology care, the department strives to manage all types of urological disorders depending upon their medical condition & is well equipped to treat a wide variety of urological diseases involving the adrenal gland, kidney, ureter, urinary bladder and prostrate. In order to minimize patient morbidity and improve recovery, the procedures are performed with the help of latest endourology techniques. TURP – Transutheral Resection of the Prostrate (TURP) is a surgical procedure to deal with patients with an enlarged, benign (non cancerous) prostrate, called Benign Prostrate Hyperplasia (BPH). When men grow older the prostate sometimes gets large enough to put pressure on the urethra, causing problems such as difficulty in urinating, incomplete emptying of the bladder or dribbling of urine. This is known as benign prostatic hyperplasia (BPH)

TURP helps a patient to remove some of the enlarged prostate gland so that he does not have difficulty in urinating. It is performed by visualising the prostate through the urethra and removing tissue by electrocautery or sharp dissection. This is considered the most effective treatment for BPH. This procedure is done with spinal or general anesthesia.

Bladder Tumour – A bladder tumour is an excess growth of cells that line the inside of the bladder. A tumour may be either cancerous or non cancerous.

Transplantation Services

A kidney transplant is an operation that replaces your kidney and places a healthy kidney in your body. The transplanted kidney takes over the work of the two kidneys that failed, and you no longer need dialysis.

Understanding your Kidneys: The kidneys are two bean-shaped organs located toward the back of the body on either side of the spine near the waistline. They are about the size of a fist and are protected by other organs and two of the lower ribs. Normal functioning kidneys serve the body in several very important ways like

  • Clean your blood and remove waste products.
  • Balance water and salt to control fluid in the body.
  • Control blood pressure.
  • Help make red blood cells and strong bones.
  • Control the amount of potassium, calcium, magnesium and phosphorus in the blood.

The symptoms of kidney failure vary widely by cause of the kidney failure, severity of the condition, and the other body systems that are affected.

  • Most people have no symptoms at all in the early stages of the disease, because the kidneys are able to compensate so well for the early impairments in the their function. Others have symptoms that are mild, subtle, or vague.Generally, obvious symptoms appear only when the condition has become severe or even critical.
  • Kidney failure is not painful, even when severe, although there may be pain from damage to other systems.
  • Some types of kidney failure cause fluid retention. However, severe dehydration (fluid deficiency) can also cause kidney failure.
  • Fluid retention – Puffiness, swelling of arms and legs, shortness of breath (due to fluid collection in the lungs, called pulmonary edema).
  • Dehydration – Thirst, rapid heart rate (tachycardia), dry mucous membranes (such as inside the mouth and nose), feeling weak or lethargic.

Common symptoms of kidney failure and end-stage renal disease include the following:

  • Urinating less than usual
  • Bleeding – Due to impaired clotting, from any site
  • Easy bruising
  • Fatigue & Confuion
  • Nausea, vomiting
  • Loss of appetit
  • Pain – In the muscles, joints, flanks, chest
  • Bone pain or fractures
  • Pale skin (from anemia)

Nephrology

The department of Nephrology offers services in the areas of hypertension, chronic kidney disease, dialysis, kidney transplantation and renal diseases.

Patients are referred to nephrology specialists for some specific disorders

  • Acute renal failure, a sudden loss of renal function
  • Chronic kidney disease, declining renal function, usually with an inexorable rise in creatinine.
  • Laser Holimium Lithotripsy
  • Hematuria, blood loss in the urine
  • Proteinuria, the loss of protein especially albumin in the urine
  • Kidney stones, usually only recurrent stone formers.
  • Chronic or recurrent urinary tract infections
  • Hypertension that has failed to respond to multiple forms of anti-hypertensive medication
  • Electrolyte disorders or acid/base imbalance
  • Diseases of the Bladder and prostate such as malignancy, stones, or obstruction of the urinary tract.

Nephrology concerns the diagnosis and treatment of kidney diseases, including electrolyte disturbances and hypertension, and the care of those requiring renal replacement therapy, including dialysis and renal transplant patients

Conditions treated under General Nephrology at Fortis Hospitals

  • Acute renal failure, a sudden loss of renal function.
  • Chronic Renal Failure: is irreversible kidney failure. The causes for chronic renal failure are diabetes mellitus (the most common cause), hypertension, long-term usage of painkillers, certain genetic diseases,
  • Chronic kidney disease, declining renal function, usually with an inexorable rise in creatinine.
  • Hematuria, blood in the urine.
  • Proteinuria, the loss of protein especially albumin in the urine.
  • Chronic or recurrent urinary tract infections.
  • Hypertension that has failed to respond to multiple forms of anti-hypertensive medication or could have a secondary cause.

Reasons for kidneys failure

Most kidney diseases attack the nephrons, causing them to lose their filtering capacity. Damage to the nephrons can happen quickly, often as the result of injury or poisoning. But most kidney diseases destroy the nephrons slowly and silently. Only after years or even decades will the damage become apparent. Most kidney diseases attack both kidneys simultaneously.

The two most common causes of kidney disease are diabetes and high blood pressure. People with a family history of any kind of kidney problem are also at risk for kidney disease.

Kidney failure occurs when the kidneys partly or completely lose their ability to filter water and waste from the blood. The buildup of toxic substances normally removed from the body by the kidneys can cause dangerous health problems.

Acute kidney failure is also referred to as renal failure and happens rapidly.

Mild kidney dysfunction is often called renal insufficiency. Acute kidney failure occurs in about 5% of people who are hospitalized for any reason. It is even more common in those receiving intensive care.

Chronic kidney failure results when a disease slowly destroys your kidneys. However this occurs over many years, and you might now realize the damage as this usually with no symptoms until the late stage of kidney failure. Progression may be so gradual that symptoms may not occur until kidney function is less than one-tenth of normal.

Generally, humans can live normally with just one kidney, as one has more functioning renal tissue than is needed to survive. Only when the amount of functioning kidney tissue is greatly diminished will chronic kidney disease develop. Renal replacement therapy, in the form of dialysis or kidney transplantation, is indicated when the glomerular filtration rate has fallen very low or if the renal dysfunction leads to severe symptoms.

Colonoscopy is a procedure that allows the physician to view the entire length of the large intestine with a long, flexible, lighted tube, called Colonoscope. It is introduced through anus. The Diagnostic Endoscopy procedures can help identify abnormal growth, inflamed tissue, ulcers and bleeding.
Key Hole Surgery

The recent upsurge in the practice of laparoscopic surgery and other forms of ‘minimal access surgery’ has ushered in a new era of surgical treatment which is having profound effects on surgical management across the various specialities
SILS : Single Incision Laparoscopic Surgery

Since the evolution of Laparoscopic surgery, it has been an endeavour worldwide to further minimize surgical trauma leading to, not only cosmetically more aesthetic scars, but also to much quicker recovery of the patient and shorter return to work times. SILS is also known as Single port access (SPA) surgery, also known as laparo endoscopic single-site surgery (LESS)

Globally, Single Incision Laparoscopic Surgery has only gained popularity and surgical technique is mainly offered at major laparoscopic centers. In India, the procedure is relatively new as most hospitals lack the expertise and equipment required for the procedure.

SILS is an advanced minimally invasive surgical procedure in which the surgery is done through a single entry point i.e. the navel. A 20mm incision at the umbilicus is all that is required for the surgery. A scope is inserted that can be moved around in the body and return images from various angles giving the surgeons a larger field of vision for the surgery and the instruments used to perform the surgery are flexible thereby enabling extra reach and maneuverability.

SILS is an excellent option for all patients who need laparoscopic surgery more so for uncomplicated Gallstones, Appendicitis and Hernia operations. At Fortis Hospital, Seshadripuram, as a routine, SILS is being offered to all suitable patients.

Hepatobilliary Surgery

  • Hepatic Resections for Benign/Malignant Liver.

  • Gall Bladder Surgery for Portal Hypertension.

Normally, blood is carried to the liver by a major blood vessel called the portal vein. If blood can’t flow easily through the liver because of cirrhosis or block in the portal vein, the pressure in the portal venous system increases. This higher pressure in the portal vein is called portal hypertension.

When the pressure becomes too high blood backflows to the veins in the esophagus and stomach (esophageal gastric varies). Beyond a critical pressure these varies rupture and gives rise to blood vomiting called Haematemesis. This requires management by a team of trained Gastroenterologist, an Interventional Endoscopist and Surgeon.
